Elevate Access Program

Because care shouldn’t be

out of reach

At Elevate Health, we believe everyone deserves access to quality care. Through our Elevate Access Initiative, we offer one complimentary occupational therapy appointment each month to an individual who may otherwise be unable to access support due to financial or location barriers. This is our way of giving back to the community — with no expectations, no obligations, and no strings attached.

What's Included

  • One complimentary occupational therapy appointment

  • Delivered in-home and/or at an equipment store (depending on referral request) 

  • A functional assessment focused on the person's specific needs 

  • Practical recommendations to improve safety, independent and participation in everyday lige

  • Detailed reporting outlining recommendations made in the appointment

Who This Is For

This initiative is best suited for individuals who may be experiencing barriers to accessing occupational therapy, such as: 

  • Financial hardship or inability to access private services

  • Limited access to occupational therapy in their local area

  • People who may not currently be eligible for funding supports (e.g. NDIS or SAH)

  • Situations where early support could make a meaningful difference to safety, independence or quality of life

How It Works

  1. A referral is submitted using the form below

  2. Our team reviews all applications each month

  3. One individual is selected to receive the complimentary appointment

  4. We contact the individual directly to arrange the appointment

As we receive multiple referrals each month, we carefully consider each submission based on need, potential impact, and suitability for the service. While we may not be able to support every application, we will always do our best to guide you toward alternative supports where possible.

We see every day how timely occupational therapy can change someone’s ability to live safely and independently at home.

Unfortunately, access to services is not always equal.

The Elevate Access Initiative was created to help bridge that gap — even in a small way — by offering direct, practical support to someone who might otherwise go without it.
