Empowering everyday life at home,
your way
Elevate Health provides mobile NDIS occupational therapy across Perth, supporting participants to live safely, independently, and confidently in their own homes. Our occupational therapists deliver practical, in-home assessments and recommendations that align with individual NDIS goals, focusing on functional outcomes that make everyday activities easier and safer.
How Occupational Therapy Supports NDIS Participants
Occupational therapy under the NDIS supports individuals to maximise independence, safety, and participation in daily life. At Elevate Health, our occupational therapists work with participants, families, and support coordinators to assess functional capacity, identify barriers within the home environment, and recommend supports that improve quality of life and long-term outcomes.

Functional Capacity Assessments are a core NDIS service provided by Elevate Health. Our OTs complete comprehensive in-home assessments to evaluate a participant’s physical, cognitive, and functional abilities across daily activities. Findings are documented in detailed reports that support funding applications, reviews, and planning decisions, clearly linking recommendations to functional need and NDIS goals.

Elevate Health provides NDIS assistive technology assessments to determine appropriate equipment that enhances safety, mobility, and independence. This may include mobility aids, seating, pressure care equipment, beds, lift chairs, bathroom equipment, and daily living supports. Where appropriate, equipment trials are completed to ensure suitability prior to recommendation.

Our occupational therapists assess the home environment to identify modification needs that reduce risk and improve accessibility. Recommendations may include bathroom modifications, access solutions, internal mobility supports, and safety enhancements. All recommendations are clinically justified and aligned with NDIS guidelines and participant goals.
