Our OTs complete comprehensive in-home assessments to evaluate a participant’s physical, cognitive, and functional abilities across daily activities. Findings are documented in detailed reports that support funding applications, reviews, and planning decisions, clearly linking recommendations to functional need and NDIS goals.

​Elevate Health provides NDIS assistive technology assessments to determine appropriate equipment that enhances safety, mobility, and independence. This may include mobility aids, seating, pressure care equipment, beds, lift chairs, bathroom equipment, and daily living supports. Where appropriate, equipment trials are completed to ensure suitability prior to recommendation.
