Support at Home – the new program replacing Home Care Packages, in effect since

November 1st 2025

What is Support at Home?

Support at Home is a government initiative introduced to update and replace the Home Care Packages (HCP) funding model. It was developed in response to the Royal Commission into Aged Care Quality and Safety and brings together existing in-home aged care programs into a simpler, fairer system designed to help older people remain living at home for longer.Under Support at Home, funding may be accessed through different pathways depending on individual circumstances:

  • Assistive Technology & Home Modifications (AT-HM): Covers equipment, products, and home modifications to support safety and independence.

  • Restorative Care Pathway: Focuses on maintaining or improving independence and function through time-limited support from allied health professionals, including Occupational Therapy.

  • End-of-Life (EOL) Pathway: Provides additional support and funding to help people remain at home during the last months of life.

 

Our Occupational Therapists play a key role in helping you maximise outcomes across all of these funding pathways.
